Covid-19 Cancellation Policy The pandemic has been stressful for us all. I am fully vaccinated and maintaining a safe environment for your treatment. For our mutual protection, masks are required for your appointment.

You may reschedule your appointment after taking 2 negative tests.

- Tests positive for COVID-19 or has been in close contact with someone who tests positive or has symptoms.
- Has symptoms of COVID-19 which may include:
- Fever or chills
- Cough
- Shortness of breath or difficulty breathing
- Fatigue
- Muscle or body aches
- Headache
- New loss of taste or smell
- Sore throat
- Congestion or runny nose
- Nausea or vomiting
- Diarrhea
